NO SHOW POLICY:

NO SHOW & CANCELLATION POLICY

(Price increase effective April 1, 2025)

Our goal is to provide high-quality, personalized medical care in a timely manner. Missed appointments, late cancellations, and no-shows limit access for other patients who need care. To ensure we make the best use of our schedule and support your success in our program, we have the following policy:

New Patients

  • Late Cancellation Fee: $100 for cancellations or reschedules made less than 24 hours before the appointment.

  • No-Show Fee: $150 for missed appointments without prior notice (no call/no show).

  • Fees will be charged to the credit card on file, and you will need to complete payment before rescheduling.

Established Patients

  • Late Cancellation Fee: $50 for cancellations or reschedules made less than 24 hours before the appointment.

  • No-Show Fee: $75 for missed appointments without prior notice (no call/no show).

  • Fees will be charged to the card on file.

Dismissal from Aspire Health

  • 2 late cancellations or no-shows within a 12-month period may result in dismissal from Aspire Health, PLLC.

  • You will receive a notification, and this will be documented in your medical record.

Late Arrivals

  • To keep appointments running smoothly, late arrivals may receive a shortened visit.

  • If you arrive more than 10 minutes late, you may not be seen and will be considered a no-show.

Telemedicine Option

  • If you can't make it to the office, we offer telemedicine appointments to help you avoid no-show fees.


FINANCIAL POLICY:

Health Insurance:

A medical insurance policy is a contract between you and your insurance company. Coverage depends upon your insurance company and the specific plan you have chosen. Aspire Health is contracted with most insurance companies and as a courtesy we agree to submit your claims directly to them.


Credit Card Payment Will Be Collected at Time of Service:

All co-pays, co-insurance, deductibles, and any outstanding balance will be charged to the card on file at the time of service or within a few days of your appointment. If you have any questions about your patient responsibility, your health insurance benefits, or if you would like a statement, please ask to speak to our Biller. We also accept cash or checks.
